Many people believe GLP-1 medications work simply by slowing digestion. While they do affect the stomach, that's only part of the story.

Their primary effect is on the brain.

GLP-1 receptor agonists activate areas of the hypothalamus and brainstem that regulate appetite and satiety. They also influence the brain's reward pathways, making food feel less rewarding while increasing feelings of fullness after eating. This reduction in appetite is considered the primary driver of weight loss, while delayed stomach emptying plays a supporting role.²–⁴

Interestingly, research shows the stomach gradually adapts to this slowing effect over time, yet weight loss continues. This tells us that the long-term benefits of GLP-1 medications are largely driven by changes in the brain rather than persistent delayed gastric emptying.¹,⁵

Why Do GLP-1 Medications Cause Reflux and Bloating?

Although delayed gastric emptying isn't the primary reason these medications work, it can contribute to digestive symptoms, particularly during the first weeks of treatment or after dose increases.⁴,⁵

When food remains in the stomach longer than usual, several things can happen:

  • The stomach stretches as it holds food for a longer period.

  • Pressure inside the stomach increases.

  • Food and stomach contents remain closer to the lower esophageal sphincter (LES), the muscular valve that separates the stomach from the esophagus.

  • Gas produced during digestion has more time to accumulate, leading to bloating and abdominal discomfort.

For some people, this increased pressure can temporarily overwhelm the LES, allowing stomach contents to move upward into the esophagus and causing reflux symptoms.⁵,⁶

This is why many people describe feeling:

  • unusually full after small meals

  • bloated or distended

  • nauseated

  • uncomfortable lying down after eating

  • increased heartburn or regurgitation

These symptoms don't necessarily mean your stomach is producing too much acid.

Acid Isn't Always the Problem

One of the biggest misconceptions about reflux is that it's caused by excessive stomach acid.

In reality, your stomach is supposed to produce acid. Stomach acid plays an essential role in digesting protein, absorbing nutrients, and protecting against harmful bacteria.⁶

The problem is often where the acid goes, not how much acid is present.

If the lower esophageal sphincter isn't keeping stomach contents where they belong, acid can reach the esophagus and cause burning, irritation, or regurgitation.
