Some people with reflux never feel a thing in their chest. What they get instead is a throat that will not behave: the constant clearing, the voice that gives out by mid-afternoon, the maddening sense that something is stuck back there that will not swallow away.

That is silent reflux, known clinically as laryngopharyngeal reflux, or LPR. It is called silent because roughly 60 percent of people with LPR do not have the typical reflux markers of heartburn or regurgitation at all, according to UW Medicine. The reflux is happening. The alarm bell just never rings.

This is also why silent reflux gets missed for years. People cycle through allergy medications, inhalers, antibiotics for a throat infection that was never there, and speech therapy, all while the actual driver sits one floor below. In one review, patients attended an average of 10 specialist visits before receiving a proper diagnosis.

Note before we go further: this guide is about adults. Infant reflux is a genuinely different clinical picture and deserves its own conversation with a pediatrician.

What Is Silent Reflux?

Laryngopharyngeal reflux is the movement of gastric contents, including acid and pepsin, beyond the esophagus into the larynx and pharynx. In plainer terms: stomach contents travel all the way up past the esophagus and reach your throat and voice box.

Ohio State describes the mechanical failure as the upper esophageal sphincter not closing properly, which is a useful distinction. Standard reflux is a lower esophageal sphincter story. Silent reflux is a story about a second gate, higher up, that is supposed to be the last line of defense.

And here is the piece that surprises most people: Ohio State also notes you can have LPR but not GERD, and vice versa. These are overlapping conditions, not the same condition wearing different clothes. Why It Is Called Silent

The throat and the esophagus are not built the same way, and they do not fail the same way.

Your esophagus has real defenses. It has peristalsis, the wave of muscle contraction that sweeps refluxed material back down. It has saliva arriving from above to neutralize what is left. It has a mucosa accustomed to occasional acid exposure. As the University of Rochester puts it, the esophagus is less sensitive than the throat.

Your larynx has almost none of that. It is not designed for acid exposure at any dose, and it does not have a clearance mechanism to speak of. Research has also found reduced carbonic anhydrase subtype III in the laryngeal tissue of LPR patients compared with healthy controls, meaning less local buffering capacity where it matters most.

The practical consequence: it takes far less exposure to injure the larynx than the esophagus. In the same review, even a single episode of pH below 4 in the laryngopharynx is considered significant, while intermittent leakage into the lower esophagus is treated as normal physiology.

So you can have enough reflux to inflame your voice box and nowhere near enough to make your chest burn. The silence is not evidence that little is happening. It is a difference in where the damage lands.

Signs and Symptoms of Silent Reflux

LPR symptoms cluster in the throat and airway rather than the chest. UW Medicine and Ohio State list:

  • Chronic throat clearing, often the very first symptom and the one people notice least in themselves

  • Hoarseness or a rough, strained voice, sometimes worse in the morning

  • Globus sensation, the feeling of a lump in the throat that does not go away when you swallow

  • A persistent dry cough with no infectious cause

  • Excess mucus or phlegm, and the sense of post-nasal drip

  • Sore or raw throat that is not accompanied by illness

  • Difficulty swallowing, or occasional choking episodes

  • Voice fatigue, particularly for teachers, singers, and anyone who talks for a living

  • Waking with a gasp or sudden breathing difficulty at night

Hoarseness is the standout. According to StatPearls, LPR contributes to hoarseness in up to 55 percent of affected patients, and nearly 100 percent of LPR patients report hoarseness at presentation, while virtually no GERD patients do. A separate review estimated that up to 50 percent of all patients with hoarseness and voice disorders may have significant LPR.

The NIDCD offers a useful timing clue: reflux-related hoarseness is often worse in the morning and improves through the day. Their description of the experience is exact, that patients may have no heartburn with LPR, but may feel as if they constantly have to cough to clear their throat.

What Causes Silent Reflux? The Pepsin Story

This is the part worth peeling back the layers on, because it changes what "treatment" should even mean.

Everyone talks about acid. But the refluxate is not just acid. It also carries pepsin, the enzyme your stomach uses to break down protein. Pepsin is secreted as inactive pepsinogen and activated by low pH, working best at around pH 1.5 to 2.

Here is the twist. Pepsin remains structurally stable until at least pH 8. Raising the pH does not destroy it. It only inactivates it reversibly, and it can be reactivated if the pH drops again. NIH literature states plainly that pepsin plays a critical role in the pathophysiology of LPR while playing a minimal or unknown role in GERD.

What happens next is the mechanism that explains everything else. Pepsin has been found in the laryngeal epithelial cells of LPR patients but not in controls. And even when the refluxate is pH-neutral, inactivated pepsin is taken up into those cells and later reactivated inside them, leading to mitochondrial and cellular damage. A narrative review describes the same process: pepsin taken up through receptor-mediated endocytosis at neutral pH and reactivated by intracellular structures such as Golgi bodies and lysosomes.

Read that again, because it is the whole ballgame. Neutralizing the acid does not neutralize the pepsin. It gets into the tissue anyway and switches back on from the inside.

Bile acids add a second layer. They are active between pH 5 and 8, which is to say they are active precisely in the range that acid suppression creates.

One more contributing factor: timing. StatPearls notes that LPR patients typically experience upright, daytime reflux and maintain healthy esophageal motility, unlike the classic nocturnal GERD pattern. This is one reason "just elevate the head of your bed" helps less than expected for silent reflux. The reflux is happening while you are vertical.

The Uncomfortable Truth About PPIs and Silent Reflux

If pepsin does the damage and pepsin survives neutralization, you would predict that acid-suppressing drugs would underperform for LPR. That is exactly what the best available evidence shows.

The TOPPITS trial, a UK multicentre randomised placebo-controlled study, enrolled 346 participants with persistent throat symptoms and gave them either lansoprazole or placebo for 16 weeks. The result: the lansoprazole group scored **1.9 points worse** on the Reflux Symptom Index than placebo at 16 weeks (95% CI -0.3 to 4.2, p = 0.096). Mean RSI was 17.4 on lansoprazole versus 15.6 on placebo. Normal RSI was achieved by 41 percent on lansoprazole and 45 percent on placebo.

Both groups improved substantially over time, from a mean RSI of 22 at baseline to 16 at four months and 15 at twelve months, with no difference in symptom improvement between the drug and placebo arms. The authors concluded that the common practice of treating these patients with PPIs is ineffective.

TOPPITS was not an outlier. A prior meta-analysis of seven placebo-controlled trials covering 396 participants also found PPI therapy lacked evidence of efficacy in suspected LPR, with a high placebo response rate. The University of Iowa Head and Neck Protocols cite a 2017 study finding that PPI therapy is not significantly more effective than a wholly dietary approach.

Two honest caveats. First, plenty of clinicians and older uncontrolled studies report good PPI response rates in LPR, some as high as 80 percent at three months. When uncontrolled studies show 80 percent response and controlled trials show none, the most likely explanation is that both arms were going to improve anyway. Second, none of this means acid suppression is useless for everyone. If you have coexisting erosive GERD, it may be genuinely necessary. It just should not be the entire plan for a throat problem.

How Silent Reflux Is Diagnosed

There is no single definitive test, which is part of why diagnosis takes so long. Clinicians generally combine three things.

The Reflux Symptom Index (RSI). A nine-item questionnaire scored 0 to 5 per item, for a maximum of 45. Worth knowing: the sources genuinely disagree on the cutoff. StatPearls uses above 10, a narrative review uses above 13, and the TOPPITS investigators cite an upper limit of normal of 12. Somewhere in the low teens is the honest answer.

The Reflux Finding Score (RFS). What the doctor sees on laryngoscopy, scored across eight findings including vocal fold edema, posterior commissure hypertrophy, and thick endolaryngeal mucus, for a maximum of 26, with a score above 7 considered abnormal.

pH monitoring. Oropharyngeal or dual-probe testing, where an LPR event is registered when pH drops below 4, with acid exposure time above 1 percent over 24 hours.

A newer option is salivary pepsin testing, with a reported optimal cutoff of 29.62 ng/mL, sensitivity 51.8 percent and specificity 93.6 percent. High specificity, modest sensitivity: a positive result means something, a negative result does not clear you.

Worth being candid about the state of the field. The TOPPITS investigators noted there is little evidence linking the symptom items of the RSI with any demonstrable reflux, and in a survey referenced in the literature, only one-third of physicians felt confident in their LPR knowledge. If you have felt like nobody could give you a straight answer, that is not your imagination.

What Actually Helps

Because the mechanism is mechanical and enzymatic rather than purely acidic, the measures that hold up best are the ones that reduce how much material reaches the throat in the first place.

  • Stop eating 3 to 4 hours before bed. The University of Iowa protocols specify no food for 3 to 4 hours before retiring.

  • Elevate the head of the bed. Iowa puts the useful range at 4 to 11 inches, with 8 inches a customary average, using blocks under the bed legs rather than stacked pillows, which bend you at the waist and can make things worse.

  • Consider alginates. These form a physical raft that prevents contact between refluxed content and the mucosa, with no significant adverse effects reported. For a barrier problem rather than an acid problem, a barrier makes sense.

  • Reduce abdominal pressure. Iowa specifically flags tight waistbands and belts, and prolonged bending, stooping, gardening, and lifting.

  • Voice care. Iowa notes there is evidence supporting voice therapy for patients with vocal symptoms. Throat clearing is itself traumatic to the vocal folds, so breaking that habit matters.

  • Stop smoking, and moderate the obvious dietary triggers, particularly high-fat meals, which slow gastric emptying.

When to See a Doctor

The NIDCD advises seeing a clinician for hoarseness lasting more than three weeks. That is a firm threshold, not a soft suggestion, because persistent hoarseness has other causes that need to be ruled out.

Get evaluated promptly for difficulty or pain with swallowing, unexplained weight loss, a neck lump, coughing up blood, or breathing difficulty. A laryngoscopy is quick, well tolerated, and answers questions that no amount of internet reading will.

A Mechanism-First Approach

If your throat symptoms have outlasted several rounds of acid suppression, the sensible conclusion is not that you need a stronger acid blocker. It is that acid was probably not the whole problem.
